Site must be located a maximum of ½ mile from another certified LHP participant
Project must be shovel-ready (e.g., grass removed, weed control implemented)
Must be able to pick up the plants in Stayton on October 10th or 11th 2025
Must provide a 1-year update including a photo, project description, and feedback
